All Health Documents

  • All documents are due by June 15 – students will not be able to join the program if this deadline is not met
  • Include name, date, provider, vaccine name/test results
  • Records must be physically or electronically signed, stamped, or authorized by health care provider (no fill labels, receipts, or insurance claims forms – these are not official records)
  • Must be typed or electronic (no hand written records)
  • No photographs or screen shots; must be high resolution print-quality scan or electronic record
    • Some examples of acceptable formats include:
  • State immunization registry (preferred)
    o UNC Campus Health Immunization Form (transcribed from handwritten records and authenticated by signature or stamp of healthcare provider where indicated)
    o Records from undergraduate health services center
    o Output from online health portal (portal-generated PDF) – e.g., pediatrician or primary care provider
    o Official copy of high school transcript

Hepatitis B Immunity
• These vaccinations are normally administered during childhood in the United States.
• If the vaccine series was not received during childhood, this requirement will take up 7 months to complete (initial three vaccine series and titer)– check your health documentation when applying and begin the vaccination series by November 15 if needed.
• Documentation for this requirement must include both the three vaccine series and a positive titer (immunity blood test) lab report with reference range/index.
• Achieving a positive titer may require additional time-sensitive steps. See the flow chart checklist for step-by-step instructions regarding this requirement.

Measles, Mumps, Rubella (MMR)
• These vaccinations are normally administered during childhood in the United States.
• Documentation for this requirement must either show two (2) combined MMR vaccinations OR individual vaccinations for measles (2 doses), mumps (2 doses), and rubella (1 dose) OR sufficient combination thereof; OR
• Positive titer (immunity blood test) lab reports for each disease are also acceptable forms of documentation
Tetanus/diphtheria/pertussis
• These vaccinations normally administered during childhood in the United States.
• Documentation for this requirement must show a minimum of three vaccinations. These vaccinations may take any of the following forms:
o DTP (diphtheria, tetanus, pertussis)
o DTaP (diphtheria, tetanus, acellular pertussis)
o DT (diphtheria, tetanus)
o Td (tetanus, diphtheria)
o Tdap (tetanus, diphtheria, acellular pertussis)
• At least one of the three required vaccinations MUST be an adult Tdap vaccine (brand name Adacel or Boostrix) given in the past 10 years. This dose of Tdap is a one-time requirement for all health science students. A Td booster is required every 10 years thereafter.
• Students must have current tetanus documentation (vaccination within the last 10 years) to matriculate.
Varicella (chicken pox)
• These vaccinations are normally administered during childhood in the United States.
• Documentation for this requirement must either show two varicella vaccinations; OR
• Positive titer (immunity blood test) lab report
• Providing a document that shows the date(s) that you had the disease is not sufficient.
Polio
• These vaccinations are normally administered during childhood in the United States.
o Documentation for this requirement must show a minimum of three vaccinations. Students who are 18 years of age or older and do not have a vaccination record for this requirement will need to contact the School after receiving an admissions offer for instructions.
Tuberculosis (TB)
• Two complete TB skin tests are required (see exception* below). This will require 4 health care provider visits total:
o Placement of first test
o Reading of first test 48-72 hours after placement
o Placement of second test
o Reading of second test 48-72 hours after placement
o PLEASE NOTE: There should be 1-3 weeks between each complete test. If there is less than 1 week between each test or if test is read earlier than 48 hours or later than 72 hours, it will have to be repeated.
• Documentation for this requirement must show the place date, read date, and results in millimeters of induration for each test.

• *If a student has ever had a previous positive skin test or cannot complete skin tests due to other contraindications they will need to contact the School after receiving an admissions offer for instructions.
