As a new Pharm.D. student, you will be required to submit health documentation to both Campus Health and Eshelman School of Pharmacy. The New Pharm.D. Student Canvas site will be shared with all new students and outlines various requirements and deadlines that need to be completed.

Frequently Asked Questions

My health records are all hand written documents that are signed by my health care provider(s).  What can I do to obtain acceptable documents?

We require students to have their information transcribed to the UNC Eshelman School of Pharmacy Immunization Form. This document must be signed or stamped by a healthcare provider to be authenticated.

Where can I find a scanner to use to scan my documents?

Some examples are: college, university, and public libraries; community pharmacies and big box stores with photo shops, office supply stores, and one stop shipping stores.

Do I also need to submit my documents to UNC Chapel Hill’s Campus Health Services (CHS)?

Yes, if you are attending UNC Chapel Hill for the first time or need to update your current records.  If you are attending UNC Chapel Hill for the first time, you will be able to submit your records to CHS in the spring after you are matriculated in UNC’s student records.  We will notify you when this happens.

Do I have to submit my health documents to more than one place?

Yes, Chapel Hill based students must provide health documents to both CHS (if you are attending UNC for the first time or need to update your documentation) and the UNC Eshelman School of Pharmacy.  Asheville based students must submit health documents to both the UNCA Health and Counseling Center (if you are attending UNC for the first time or need to update your documentation) and the UNC Eshelman School of Pharmacy.

Where can I find instructions on how to submit my documentation to CHS or the UNCA Health and Counseling Center?

If you will be a Chapel Hill based student, you can find them here. Remember, if you are attending UNC Chapel Hill for the first time, you will be able to access Connect Carolina and submit documents to CHS after you are matriculated in UNC’s student records.  We will notify you when this happens.

Can a friend of family member call or email on my behalf to ask about my health records?

Federal law only allows us to discuss your health records with you.  You will be using these records throughout your career as a health care professional, so it is best for you to lead the way in managing them from this point on.

I would like to show someone at the School my health records before my official submission.  How can I provide them for a review?

Please email Campus Health at immunizations@unc.edu, and they will provide further instructions.  For security reasons, please do not send them via email.

In addition to completing a full hepatitis B immunization series, you must have a quantitative hepatitis B titer that shows immunity to the virus.

My Hep B titer results were negative for immunity.  What do I do?

Don’t panic!  Open and save the Hep B Immunity Flow Chart to see what your next steps are.  You will need to do a few more things to ensure you have the highest level of immunity possible.

My Hep B titer results were negative for immunity.  Will that jeopardize my acceptance into the program?

No, you will still be able to join the program.  You will need to follow the steps outlined in the Hep B Immunity Flow Chart.  We encourage you to email Campus Health at immunizations@unc.edu if you are uncertain how to proceed.

May I use my childhood Hep B vaccinations to fulfill the vaccination part of the Hep B requirement?

Yes, you can use your childhood vaccinations for the vaccination part.  Don’t forget to also have your titer performed!

Do I need to have both the Hep B vaccination series and positive titer to fulfill the Hep B requirement?  Can I use one or the other?

Yes, you will need to have both.  Just one will not be sufficient.

I have tested positive for TB with a previous test.  What should I do to complete the program’s TB requirements?

I received the BCG vaccine.  What should I do to complete the program’s TB requirements?

I received a chest x-ray as part of an earlier evaluation after a positive skin test.  Do I need to have another performed?

No, you do not need another x-ray performed unless directed by UNC Campus Health Services (CHS). Please email Campus Health at immunizations@unc.edu for instructions.

An individual attending school who has attained their 18th birthday is not required to receive polio vaccine.
